Child travel with grandparents

edited December 2016 in Uncategorized Topics

  • Yes you will need an affidavit or consent allowing your child to travel. A bit tough sa pinas travelling with non immediate family members. A bit risky but probably your sister or brother can bring your kid here if their surname matches your kid's
  • Ok. So malabo talaga. Dapat talaga may affidavit which is need b galing pa talaga sa embassy dito?

Download mo yung form. Fill up mo. Then pwedeng yung nasa pinas na yung magprint

    2. Gawa ka letter of consent
    Should include
    - name and passport details ng minor pati nung makakasama niya.
    - travel details (date ng flight at balik)
    - contact details mo

    3. Ask someone na pumunta sa dswd, dapat dala yung form at consent letter. Kahit wala signature ok lang. O e-signature pwede din.
    - birth cert nso. Original
    - passport photo ng minor
    - 300 for 1 year, 600 2 years
    - marriage cert
    4. Magdala din ng copies ng id mo

If maaga makapunta same day lang din makukuha.
