Mismatched name in Birth Certificate and Passport vs Diploma, COE, etc.

Mga ma'am, sir,

I need your advice and opinion po. I got a job offer na po and the company is asking for my birth certificate, passport, COE, diploma, and transcript.

Ang problema ko po, dahil illegitimate child ako, ang last name ko po sa birth certificate ay sunod sa mother ko. Pero sa diploma and transcript at COE (and other PH employment docs) ay sunod po sa last name ng father ko.

Wala naman daw po problema sa company, pwede daw po nila ako i-hire na sunod sa last name ng mother ko.

Ang tanong ko po, magkaka issue po ba ako sa MOM/E-PASS application?

Sobrang kinakabahan po ako at sobrang manghihinayang kasi sure na po yung job offer pero ayun nga, naka depende po talaga sa e-pass.

Balak ko po mag attach ng Affidavit of One and the Same Person sa mga requirements ko. Uubra po ba ang ganun?
Gaano po kalaki ang chance na ma-approve/reject ako?

Hope you can help me po if na experience nyo po ang same case or pag may kakilala po kayo na ganito rin ang issue.
First time ko po ito sa SG, sorry po kung mali mali ang terms ko.

  • Yung case ko dati, misspelled middle name (diploma and passport), ung tama ung nasa passport. Nung picture taking na sa MOM, napansin ng officer, pinayagan nman ako pero inadvise sakin na ayusin ko yung papers ko once na umuwi ako ng pinas. Naayos ko nman kaya wala nang problem.

    Kung may time ka pa, ayusin mo na mga docs mo. Or, dalhin mo nlang supporting docs na magpapatunay na iisang tao lang ung naka state sa BC and passport mo.

    Medyo similar nga tayo ng case dahil may discrepancy.

    Pero tingin ko po mas halata yung sakin kasi ganito po sya
    Example:
    Birth cert & passport: Juana Reyes (no middle name, mother's last name)
    Diploma, coe, and all other docs: Juana Reyes Santos (middle name is Mother's last name + Father's last name)

    Mukhang di na aabot pag pina ayos ko pa kasi tatakbo na next week ep application ko. Eh aabot po ng taon ang pag update ng name sa birth cert, dadaan pa ng korte.

    But anyway thank you po ma'am will bring all supporting docs lile you advised.

    Sana po talaga hindi sumabit.

  • Yung sinunod ko, ung birth cert ko.. yup matagal tlaga mag process sa PSA (NSO). Yung diploma nga inabot ako ng 3 wks

    Once na nakalusot ka, (na issuehan na ng pass) ayusin mo agad ung passport and diploma mo.
  • Subukan mo kontakin ung phil embassy dito sa sg. Alam ko may certification na pwede ka mag request na mag papatunay naiisa ung tao. Like sa casw ng babae after kasal kung san nagiging part ng name ung surname ng babae ung surname ng lalake. Nakalimutan ko tawag sa certificate na un.

  • Thank you Admin! Try ko po maka kuha nyan ngayon habang in process ep ko. Kaso Hindi po ba ako magkaka problema kasi advise po sakin ng recruiter ko, lalabas daw po ako pinas as tourist and papasok as employee. Pag kinontak ko po ang embassy baka madetect nila na may balak ako mag work dun

    Yung name ko naman isang letter ang difference ng birth certificate and yung nakasanayan kong name.

    For example:
    ID, DIiploma, other docs: JULIE. yun yung name ko na ginagamit ever since so sa lahat yun ang nakapangalan.
    Birth certificate and passport: JULEE

    Narealize lng ng parents ko nung nagapply na kami ng passport nung student pa lang ako.

    So nagpagawa kami ng affidavit. Pinasa namin yun sa DFA for passport application. Yung passport ko, julee ang name. Pero yung diploma ko, julie ng nakalagay. Pinasa ko na lang din yung affidavit in case na tanungin.

    Kaso hindi pa naapply ng employer yung pass ko so i cant tell you if accept yun ng mom or not.

    Baka pwede po pakibalitaan naman ako if ano result ng sainyo.
    Yung akin po mukhang next week na sisimulan ng company ang processing.

    Kamusta po, kinakabahan din po ba kayo? Or tingin nyo po mataas naman chance na maapprove ang case nyo?

    Kinakabahan ako kasi di pa nila naapply yung pass ko...more than 1 week na hahaha.

    I think hindi nman siguro major grounds yun to reject a pass. Baka maguluhan lang sila kasi iba ang name kaya baka mas important ata talaga for us to have yung affidavit.

  • @goblinsbride follow up question lang po. So bale birth cert and passport nyo po same name na tamang name then sa diploma yung maling name?
    Nag try Pa po ba kayo mag correct ng diploma? Yung sa passport alam ko po di basta basta macocorrect kasi birt cert ang requirement dun. And hindi naman din po Basta Basta mauupdate ang passport. Tama po ba
  • Hindi pa po ako nagpapacorrect ng diploma. Napasa ko na rin kasi kaya yung affidavit na lang sinama ko.

    Mahirap sagutin kasi MOM lang makakapag sabi, pero I think ang mahalaga matched yung name sa passport at birth certificate mo, yan kasi yung primary docs na needed.Tapos yung mga supporting documents like diploma, COE, kung hahanapin ng MOM or tatanungin tsaka ka na mag provide.

    Pero dapat ayusin mo yan while maaga, kasi kung may future plans ka to migrate sa ibang countries like AUS, NZ, CAN, Euro zone, eh strict sila sa ganyan.

  • @tambay7 sir yes po matched naman ang passport and birth certificate :) and ongoing na din po ang pag aayos, medyo mahabang process lang. Medyo kinakabahan lang hearing na sobrang nag strict na si mom nowadays but still very hopeful and praying hard. Thank you po for answering :)
